Peninsula Floorball Club
Terms and Conditions of Registration and Site Use
By registering with Peninsula Floorball Club Inc, you agree to the following terms and conditions:
1. Code of Conduct
I agree to abide by the Peninsula Floorball Club Inc Code of Conduct, and unless otherwise stated, I am also bound by Floorball Australia policies.
2. Insurance
Peninsula Floorball Club registration does not include any personal insurance beyond what is provided under the Floorball Australia Limited (FAL) policy. Full details of the FAL insurance coverage—including inclusions, exclusions, and claims procedures—can be found at: https://www.floorballaustralia.org.au/policies-and-documentation/insurance.
By registering, you agree to arrange any additional insurance necessary to cover injuries, loss, or other liabilities not covered by the FAL policy. Peninsula Floorball Club Inc is not responsible for ensuring players are insured. Each member and their guardian, where applicable, must ensure they have appropriate insurance before taking the court.
3. Media Consent
By participating in Peninsula Floorball Club events, you grant the club the right to use media (photos and videos) that may include your image. If you do not consent, you must notify Peninsula Floorball Club Inc in writing.
4. Junior Players
It is the responsibility of a parent or guardian to inform the club in writing of any pre-existing injuries or medical conditions for players under 18. All players under 18 must be accompanied by a parent or guardian at all times during club activities.
5. Registration and Payment
No registration or order is valid until full payment is received. Shirt orders are subject to manufacturer processing times. All players must complete a registration form prior to participating in any Peninsula Floorball activity. By registering, you agree to pay the advertised fee for the correct division.
6. Behaviour and Conduct
By registering, you agree—on your own behalf and on behalf of any dependents—to behave in a sportsmanlike manner at all times and comply with the Peninsula Floorball Club Code of Conduct.
7. Trial Period
New players may join for a two-week trial before committing to a full season. This trial period comes without insurance cover, and Peninsula Floorball Club Inc is not liable for any injuries sustained during this time. Players are encouraged to consider arranging appropriate insurance via Floorball Australia.
8. Membership Declaration
I hereby apply for membership of Peninsula Floorball Club Inc. In applying, I acknowledge and agree to the declaration, including the warning, exclusion of liability, release, and indemnity clauses located at www.floorballaustralia.org.au.
I understand that Peninsula Floorball Club Inc may reject this application at its discretion. If accepted, I understand I will receive the benefits and services associated with the relevant level of membership, as outlined in the Peninsula Floorball Club Constitution.
